Annual Complaints Submission

The Office of the West Midlands have advised that they are not using MS Forms for gathering Annual Complaint information this year and pharmacies will not be able to access any old MS Forms. Pharmacies can submit their data via email to nhsbsolicb.pharmacy-westmidlands2@nhs.net

Some pharmacies are using an old Word template – which you can find here  – and others are just sending in the information in an email with practice details (make sure to include your ODS code).

Once the Office of the West Midlands have received your data, they will upload it to their own spreadsheet and if they have any queries, they will contact you.

There’s no official deadline, but please make your submissions as soon as possible, if you haven’t done so already.
