CD Reporting: important change regarding concerns and incidents

Please note that following a review of reporting patterns, it has been decided that the majority of submissions categorised as “concerns” should in fact be reported as “incidents”. In order to improve accuracy, streamline processes and ensure appropriate follow‑up, the Concerns module on the CD reporting website will be disabled. All future reports must be submitted as incidents. CD Reporting will continue to provide an option to submit anonymous concerns for whistleblowing purposes.
